The car behind the Flat is a RPO but what is the flat? It looks like it was used in alcohol spraying service with the 3rd rail contact shoe.
Also notice how safety was not a priority. They added a step ladder almost directly above the shoe but on the other end of the car there is no access.

The concept of safety is largely irrelevant, a stirrup over a contact shoe only means you've got to be aware of your surroundings.  No matter what "safety precautions" you take, an idiot can and will wreak havoc.
